It makes the page you are viewing easier to read: a reading ruler, paragraph focus, readable fonts and spacing, and a color overlay. It runs only on the page where you click the extension, and only when you click it. Per-site settings (Pro) are saved preferences: they never make the extension run on its own.
None. Your settings are stored locally in your browser (chrome.storage); with Chrome sync enabled, Chrome may synchronize them across your own devices under Google's privacy policy. The extension changes the appearance of the page inside your browser. Nothing is sent to us or to anyone else.
If you activate a Pro license, the key you enter is sent to Gumroad (Gumroad, Inc.) to verify the purchase, at activation and then about once a week. Gumroad's privacy policy applies: gumroad.com/privacy.
